Paid Time Off (PTO) Policy Example

Purpose of Policy:
This PTO policy outlines the provisions for paid time off accrued and utilized by employees of Acme Corporation to ensure clarity regarding leave entitlements, procedures, and usage guidelines.
PTO Accrual and Eligibility:
Employees accrue PTO at a rate of 1.25 days per month during the first year of employment, increasing after specified periods of service. PTO is earned starting on the employee’s first day of employment and is subject to applicable state and local laws.
Usage and Approval:
Employees must submit leave requests at least two weeks in advance, except in cases of emergency. PTO approval is subject to operational needs and manager discretion.
Carryover and Payout:
Unused PTO may be carried over up to 40 hours annually. At termination of employment, accrued but unused PTO will be paid in accordance with state law.
Employee and Employer Responsibilities:
